PERFORMANCE ASSESSMENT INTERVIEW
This tool helps the change agent start the data gathering process to better understand the situation at hand.
The tool outlines a series of questions to help define and evaluate the presenting situation, identify potential
ideas to go forward, and identify stakeholder-focused next steps.

ARCI (ROLES AND RESPONSIBILITIES) MATRIX
Often teams become less effective because there is no clarity of roles and responsibilities. This tool helps clarify
critical roles of a project plan or a process. This template also helps the team develop a simple work breakdown
structure for a project (or a process) to ensure the team understands what is required to achieve key
deliverables.

BEHAVIOR GAP ANALYSIS
Culture changs is rooted in human behavior. The first step to achieving lasting and meaningful behavioral
change is to identify the critical behaviors required for the change to be successful. This tool helps change
leaders and agents identify critical behavior and connect it to business outcomes. It also promotes dialog
about the reinforcers or hinderances to desired behaviors.
